Frequently Asked Questions about the Intake Process

  • Why do I have to fill out this form? The intake form helps the mediator learn about what’s going on, what everyone needs, and any special details that could affect your session. It helps the process run smoothly and fairly for everyone.
  • Is my information private? Yes, the details you provide are confidential. The mediator only uses this information to prepare for your mediation session and will not share it outside the mediation unless required by law.
  • What if I don’t understand a question? It’s okay! If you’re not sure what a question means, you can skip it or ask the mediator for help. The mediator is there to make the process clear and comfortable for you.
  • Do I have to answer every question? Try to answer as much as you can, but if you don’t know or don’t want to answer something, leave it blank and talk about it with the mediator during your meeting.
  • Will the other party see my answers? No. The intake form goes to the mediator only. You can share as much or as little as you’re comfortable with, knowing the mediator will keep it private.
  • What happens after I send in the form? The mediator reviews all the forms, checks if more information is needed, and then contacts everyone to plan the next steps. You’ll get information about the first mediation meeting.
  • What if my situation changes after I fill out the form? Tell your mediator if anything important changes. You can always update or add information before the session.
  • Who should I contact if I have questions? You should contact your mediator directly—by phone or email—if you have any questions or concerns about the intake form or the mediation process.
  • What if I have documents for the mediator to review? After you submit your form the mediator will make a document request. That request will include a secure link whereby you can upload documents to a secure space that only the mediator can access. Also, as part of your logging into the ADR Notable system you will have a Dashboard that includes a list of your case (all cases you have with us) and access to a secure upload link directly from the Dashboard.

After You Complete the Form:

  • The mediator reads all the forms from each person.
  • If something isn’t clear, the mediator will contact you.
  • You’ll get information about the next steps and the date of your session.

Your mediator uses the secure, integrated mediation platform from ADR Notable. It provides the intake process whereby you will supply the mediator with the essential information needed to have a smooth and effective mediation. As a first time user you will be asked to create an account on ADR Notable. There is no charge and it results in a private and secure Dashboard for your use in managing your case (cases if you have more than one). You will create an account with a password and only you can see your Dashboard.
